2010 AzPTA Scholarship Application Instructions

Qualifying PT and PTA students studying in Arizona CAPTE accredited programs are invited to apply for scholarship funding from the AzPTA by completing the AzPTA Scholarship Application Packet.  The applicant's Faculty Liaison is responsible for submitting all documents via fax at 480.603.4141 (required so that documents are in PDF format and automatically time/date stamped) on or before August 15, 2010. The Faculty Liaison will receive a confirmation receipt of transmitted items. Scholarship recipients will be notified by the AzPTA Scholarship Chair or his/her designee prior to the Fall AzPTA Meeting and will be invited to attend the meeting to personally receive the award. Applicant eligibility criteria:
* Applicant shall be a student at an Arizona PT or PTA program that is accredited by the Commission on Accreditation in Physical Therapy Education (CAPTE).
* Considerations for the award(s) shall be based on the submitted application documents and the applicant's grade point average (GPA); the GPA shall be submitted to the AzPTA Scholarship Committee Chair by the applicant's school.
* Applicant shall be an APTA member to be considered for funding.
* Applicant shall be in the final year of their educational program.
* It is the applicant's responsibility to ensure that the application packet is complete. Incomplete applications will not be considered.
