Purpose  
To recognize and honor an individual who has made a significant, ongoing and distinguished contribution to the Arizona Physical Therapy Association (AzPTA) through volunteerism.

 

Eligibility 
An individual who has been actively involved in AzPTA activities during the past ten years, having made a significant contribution of volunteer time, talents, and service to the mission of the AZPTA is eligible. The individual shall not currently be a member of the AzPTA Board of Directors (BOD).

 

Criteria
The Nominee shall have demonstrated exceptional, sustained service in three of the following areas:
a. Leadership: May include service as a Delegate, Committee or Task Force Chair or member, or former member of the BOD.
b. Promotion of the Profession:  Has made significant and sustained contributions to the promotion, publicity and development of physical therapy as a profession.
c. Collaboration:  In their collaboration with other professional organizations, this individual enhances other organizations’ understanding and appreciation of the mission, scope, and work of AzPTA.
d. Legislative:  Has made significant and lasting contributions to the writing, monitoring, and promotion of the physical therapy practice act to ensure safe and competent physical therapy practice.
e. Education:  Has made significant and lasting contributions to the education of physical therapy students, continuing education of physical therapy members for continued competence, or mentoring of physical therapy members toward development of the profession.

 

Procedures 
Complete the following Nomination Form describing the nominee’s activities justifying their exceptional contributions. The form requires submission of two letters of support written by other AzPTA members. A committee made up of the last three recipients and Pauline Wampler will make the selection from the nominees. Only one award will be presented each year at the Annual Fall Membership Meeting. Proviso: The President shall appoint a special task force selection committee of three members with terms of three, two and one years until the committee is self perpetuating.

 

Award
The award will be a gold pin designed exclusively for this award that will not be otherwise available. In addition, the recipient will receive a plaque acknowledging their contributions.
